Confess your worst industry fuck up so far?

We were making infused prerolls by brushing on melted disty. The disty was being heated in a non-heat safe glass jar on a cheap hotplate (someone else was in charge of that prepping that). I was lifting up the jar to clean under it when the bottom broke off and the contents fell all over the hotplate and the table. It smoked out the entire area and I got a snootfull. That was not fun to clean up, but thankfully it was only 50-100ml and I learned a valuable lesson.
